I acknowledge that I am aware of and understand the risks associated with receiving a facial treatment, and that I have had the opportunity to ask my aesthetician any questions, which have been answered to my satisfaction.

I confirm that I am voluntarily choosing to receive a facial treatment.

I confirm that I do not have any open skin lesions or an outbreak of herpes in the area where I will receive the facial.

I confirm that I am not currently under the influence of alcohol or recreational drugs.

I confirm that I have provided an accurate and truthful account of my medical and skin history, including all allergies, current medications, and topical products that I am using to my aesthetician prior to receiving my facial treatment.

I agree to release, indemnify, and forever hold harmless my aesthetician from any and all claims, damages, or legal actions arising from or connected in any way with my facial procedure.

I understand that if I experience any unusual discomfort, adverse reaction, or concern following my treatment, I must notify the service provider within 12 hours of the appointment to allow for proper assessment and support.

3. Aftercare:
You understand that proper aftercare is essential for optimal results and agree to follow all post-treatment instructions provided by your aesthetician.

4. Skin Reactions:
Temporary redness, sensitivity, or breakouts can occur following treatment. If you experience any prolonged or adverse reactions, you should contact a healthcare provider immediately.

5. Liability Waiver:
You release the skincare professional and the establishment from any liability associated with potential skin irritation or reactions that may result from the facial treatment.
